To establish a process that allows prison officials to ask the court that sentenced a prisoner to transfer the prisoner to a facility that can provide acute long term medical or mental health treatment or services, if the prisoner's physical or mental health has deteriorated to a point that renders the individual a minimal threat to society. This would not apply to prisoners sentenced to life without parole or first degree murder. The nursing home or other medical facility would not be responsible for enforcing a prisoner's parole conditions or reporting violations of them.
